The identity of those participating in WindVOiCe© is completely confidential. We use special protocols to protect the identity of survey participants.

If you wish surveys for your family, copies will be mailed out to you. You can ask for a survey for children which you can fill on their behalf.

You will receive general instructions on how to fill out the survey.

If there is more than one adult in the home being affected, each adult is required to fill out a separate survey.

Respondents have to be 18 years of age or older and have a fluency in English. Anyone with cognitive impairment cannot participate in the survey.

You can add additional comments about your experiences in the comments section.

Parents are encouraged to fill out a special survey for children. This can be provided on request.

A return address and mailing instructions are provided with each survey.
